Accidentally on purpose...I dont think so. On purpose...just a few bits of paper
By accident: I set my oven glove on fire. While cooking some bits of meat on the stove. Suddenly I thought 'hey, why are my fingers feeling warm?', and I pulled back my hand and the glove was on fire, and not barely on fire. From about 75% up, it was covered in fire. So I threw it in the sink and poured water on it. It was actually kinda fun

Dead or alive? Doesnt really matter, Ive done both. Although the alive ones were flying bugs that flew into my mouth. By the way, on average a person eats like 6 spiders a year or so (I think).
As for dead, Ive eaten crickets and grasshopper like things, and some worm like things...they looked like meal worms. All very salty and not bad.
